All applicants are advised to thoroughly review the Program and Application Guide. All application questions are included in the guide and should be reviewed before starting this online form. Applicants can only submit one Online Application Form per intake.
This online form must be completed for all applications to LGPS funding programs and includes questions that are common to all grant applications. When the form is submitted, the information will automatically be submitted to UBCM and the applicant will receive a confirmation email, including the file number and a copy of the required worksheet. Please note that this online platform does not auto-save responses.
